关于推荐信抬头

老师答应帮忙写推荐信，然后她问我抬头应该写想申请学校的具体某一个人/或者学院/或者委员会的地址信息吗？
举例：
Name of the department
Name of the institution
Address of the department
Email of the department

To whom it may concern:
想请问的是这些是些推荐人所在学校还是说想申请的学习所在学校的信息？
