i765的一个问题，part6填写过往cpt

学校给我检查的i765说我要在part6写上之前给过的cpt，我看了一下instruction，写的是
Post-Completion OPT--(c)(3)(B). File Form I-765 up to 90 days before, but no later than 60 days after, your
program end date. Use Part 6. Additional Information to provide all previously used SEVIS numbers and
evidence of any previously authorized CPT or OPT and the academic level at which it was authorized.
我应该怎么写呀，我研究生有2个CPT，都在同一个学校，sevis码和opt这个是一样的。有没有人来传授一下经验~
