STEM OPT 换雇主需要前雇主填evaluation吗

学校网上说Please complete this form with your supervisor and upload it here. You can find the link to the latest I-983 form at the bottom of this eForm. Additionally, a page 5 Evaluation is required from the employer that you are leaving. These documents should be scanned into one file only.
第五页evaluation是有雇主签名的地方，但如果没满12月，还需要做evaluation吗？
