RT。学校要求是:
Ask your medical provider or nurse to complete and sign the Immunization History form (physician’s office address and phone number must be included). In lieu of completing the form, you may attach a copy of school immunization records, physician records or military records.
International Students: You must provide a certified English translation if immunization records are not in English. Please keep a copy of your immunization records in your personal files.
那么问题就变成了：
我手上的中国的疫苗记录（一本是很小的时候用的，还有一个是前几年补的，都是中文），应该到什么地方去get a certified English translation？医院可以吗？出入境检验检疫局可以吗？
